Ascend, a MAXIMUS Company (“Ascend”), respects your privacy and we have developed the following policy to demonstrate our commitment to you.  This Privacy Policy applies to Personally Identifiable Information we collect about you on Ascend’s websites located at ascendmi.com and pasrr.com (“Sites”) and describes the types of information collected, how that information is used, and the choices users have about collection and use of this information.  This Privacy Policy does not govern privacy practices associated with offline activities, websites other than our Sites, or products and services not available or enabled via our Sites, except as expressly set forth in this Privacy Policy., 

Ascend is committed to protecting the privacy and security of Personally Identifiable Information (PII) and Personal Health Information (PHI), collectively known as Personal Information (PI), in accordance with federal and state regulations. Information regarding your health and the care you are receiving is safeguarded in accordance with the law. In general, Ascend only uses or discloses PI for maintaining our contracts with our customers, and follow the mandated state and/or federal guidelines on privacy to ensure compliance. However, in all situations Ascend uses or discloses your PHI only as permitted by law or pursuant to an authorization.

The information ASCEND collects

Although much of the content of our Sites are available without registration or providing personal information, in order to take advantages of some the features of our Sites, you will be asked to provide Personal Information.  Your Personal Information is provided voluntarily by you, the user, in order to participate or register for a particular product or service, or when you otherwise communicate with use or our Sites.  Personal Information will be collected at various pages on our Sites, including when you:
Submit an online inquiry
Submit an investor relations information request
Sign-up for email alerts, newsletters or other Ascend information
Submit an employment application

We clearly identify data you must provide so we can deliver the services or information you request.  Ascend may ask that you provide Personal Information, such as your first and last name, mailing address (including ZIP/Postal Code), country of residence, e-mail address, employer, job title, department, telephone numbers and other personal information.  In addition, if you become a user of ascendami.com or PASRR.com, Ascend may ask you to identify the company, facility or state agency that you are representing.  Providing this information is optional, but failure to do so in some cases will limit the services available to you.  For some programs available through our Sites, you will need to become a registered user.  Only properly registered and authenticated users may be authorized to submit case information, make requests for review and view previously provided case information.  
In addition, our Web servers automatically collect and log Web usage data from you when you visit our Sites.  This information, including your Internet Protocol (IP) address, referring sites, pages viewed, browser, operating system, CPU speed, referring or exit Web pages and length of visit informs us about how visitors use and navigate the site.

How We Use and Share the Information Collected

Ascend does not share your personally identifiable information to unaffiliated third parties, but we may use your information to, among other things:  

Provide Newsletters and Announcements which may be provided on a periodic basis as a service to various member groups including general announcements regarding Ascend’s services, announcements that are specific to a particular group of users on our Sites, and specific announcements to a user in response to a specific request by the user.
Surveys in which voluntary feedback is used to determine and improve Ascend’s services to its users.
Improve the content, navigation and efficiency of our Sites
Make you aware of other benefits, products, services or events offered by Ascend that may interest you
Enforce Ascend’s Terms of Use and otherwise prevent illegal or improper activity to our Sites
Notify you of changes to this Privacy Policy or to other policies that may affect you and your use our sites.

In order to respond to an information request you submit through our Sites, we may share your personal information with our affiliated companies that may be able to respond directly to your inquiry.  In addition, we may disclose your information to third parties (1) in connection with a merger, sale or consolidation of all or substantially all our assets, or (2) to comply with valid legal processes such as a search warrant, subpoena or court order.  By using our Sites, you agree and acknowledge that personal information collected through our Sites may be transferred across national boundaries and stored and processed in any of the countries around the world in which Ascend maintains offices, including the United States.

We may use and share with others aggregated or anonymous (not Personally Identifiable) information that we have collected based on Web usage data, surveys or statistical information that we have compiled about our users.

Information Collected by Your Web Browser – Cookies

Like most websites, we employ “cookies,” “web beacons” and similar devices to help you more efficiently use our Sites and to track your activities.  A cookie is a small amount of data that is transferred to your browser by a web server and can only be read by the server that gave it to you.  It functions as your identification card and enables Ascend to record your activities and preferences.  It cannot be executed as code or deliver viruses.  Cookies from Ascend can only be read by Ascend websites.  A Web beacon is a small transparent gif images that is embedded in an HTML page or e-mail used to track when the page or e-mail has been viewed.  Ascend uses cookies and similar devices to track your use of our Sites, types of Products and Services viewed and information downloaded, count visitors we receive daily, and our Web servers automatically log the IP/Internet address of your computer. Ascend may also use a “session cookie” for navigation purposes that is active only during your session.  Ascend does not generally use this information to identify you personally.  If you do not want your browser to accept cookies, you can modify the cookie option in your browser’s settings.  However, some of our Sites’ features or services may not function properly or be accessible without cookies. . 

Linked Sites and Collection of Information by Third-Party Sites and Sponsors 

Third-party sites and sponsors do not have the ability to collect personal information from the computer you are using when you are directly viewing an Ascend website. To provide you with the best services possible, Ascend collects only data about utilization of these websites and does not collect any information from third part sites. Ascend cannot control the activities of any third parties once you leave its website. Links to other sites do not imply endorsement by Ascend and are merely provided for your convenience only.  If you click on a link or banner to leave an Ascend website, Ascend’s Privacy Policy no longer applies to the new site. Some linked sites may collect information about users. Please check the banner ad's website to determine its privacy policy. Ascend is not responsible for nor does it endorse the privacy practices of its linked web sites.  However, if you have questions about the privacy policies of such third parties or the information such sites collect, Ascend suggests reviewing the site’s posted privacy policy or contacting the third parties directly. 

Children

The Site is not directed towards children under 13 years of age, nor do we knowingly collect information from children under the age of 13.

Security

Ascend has implemented commercially reasonable technical, administrative and physical security measures to protect your personally identifiable information from unauthorized access, modification or use.  We periodically review and update security measures in light of current technologies.  Please note that despite our efforts, no security measures are completely secure.  Please do not submit Protected Health Information through the public portion of our Sites.  Please contact the individual program with any questions or enrollment requests.

Account Revisions, Revoking, and Deleting Personal Information

We try to keep information about you accurate and current.  You may request to review, correct, update or delete Personal Information that you provide to Ascend. To update or correct such information and privacy references, it is Ascend’s policy that all such requests be made either in writing or via the phone.  In any written correspondence, please indicate with as much detail as possible the location on our Sites or otherwise where you provided your Personal Information. Although an email address has been provided below, please do not send any Personal Information via email as your email message may not be sent in a secure manner.  If you request to delete your information, Ascend will not use it for any purpose except where required by applicable laws, regulations or medical practice standards.
Personal Information maintained by any of our contracts administrated on behalf of federal, state or local government entities should be corrected by contacting the customer service department of the individual program.

Ascend employees

All Ascend employees, contractors and subcontractors have to read, understand and agree to follow its privacy and confidentiality policies and procedures and its security policies and procedures.

Changes to this Privacy Policy

We will occasionally update this privacy policy.  When we do, we will revise the “updated” date at the top of the Privacy Policy. Please check our Sites from time to time for the most current version of our Privacy Policy.  Your continued use of our Sites after we have notified you have changes as described above constitutes your acceptance of the changes
