Who will participate in the assessment?
Participants, also referred to as respondents, may consist of the individual being assessed as well as people who have known the individual and could include family members, friends, or direct support or other staff from residential or day service providers. The individual may choose or may be required to be an active participant in the interview, depending on the assessment.
What will happen on the day of the assessment?
A professional clinician will visit you and conduct an interview with you and/or your guardian and caregivers. The clinician (or nurse, depending on assessment), will explain how the interview works. Most likely, he or she will ask you a series of questions to assess your physical ability or personal questions related to establishing your mental health status. Don’t be nervous—Ascend clinicians are highly trained and work to help you get the support you need.
