Level II Process Change

Ascend has worked with DSS to revise our Level II assessment process. There may be times when a person needing a Level II receives a document-based review (DBR) rather than an onsite assessment. The process for this has been developed in conjunction with DSS, and these DBR's will occur under very specific circumstances when an individual is seeking additional time in the NF after a recent onsite assessment resulted in a short-term approval. Providers are not able to request DBR's in lieu of onsite assessments.

Clarification Regarding 60 day Convalescent Stay Option

The 60 day convalescent stay option is for persons with major mental illness (MMI), mental retardation (MR), or related condition (RC). To use this option, the discharging provider must complete Level of Care and Level 1 screens and select 60 day convalescence on the Level 1 form. A signed physicians's attestation that the stay will likely be 60 days or less must be submitted. The admission to the NF can occur after an Ascend Nurse approves the decision.

All Connecticut Providers:

Due to recent approval of a state plan amendment for PASRR a Level of Care (LOC) will no longer be required for 30 day Exempted Hospital Discharges (EHD).
If you have questions, please contact an Ascend Project Specialist.

Attn CT Providers: Be advised starting 7/1/11, retrospective reviews will require the approval from Ascend Manager and State Office. Individuals will not be approved retrospectively if a screen is not submitted timely. It is important to utilize WEBSTARs and tracking process to ensure timely filing.

  • SEPTEMBER TIP OF THE MONTH

    The DSS recently received federal approval for an additional option to streamline the Level II process for applicants needing Nursing Facilty care for less than 60 days. This option may now be selected as question number 25 on the Level 1 screen (remember to check the box under #21 indicating that you are seeking categorical or hospital exemption).
