Beginning Monday, July 18th, 2011, the State of Colorado will allow individuals with 60-day convalescent approvals who re-admit to the hospital for a medical reason to return to the same nursing facility without a new pre-admission screen, if they return to the nursing facility within the 60-day approved time period. The nursing facility is responsible for determining if there is enough time left in the 60-day approval to return to the nursing facility or if a new screen in needed because the initial 60 days will expire before returning. This change will prevent unnecessary delays for individuals who are psychiatrically stable and are going to the nursing facility for short term care.

Beginning 7/1/2011, the State of Colorado and Ascend Management Innovations will implement a new web-based approval process for individuals receiving Hospice services who are admitting to Medicaid certified nursing facilities. Please read the PASRR Hospice Process document in the "Evaluator Tools" section below.

  • Please review the PASRR individual's psychiatric and medical information very closely prior to completing a PASRR screen. This will ensure that all submitted information is accurate and complete. This will also save time and the need to resubmit the screen for missing or incorrect information.
