Authorization for Release of Information
Virginia Nursing Facilities Tracking Form
Nursing Facilities are responsible for notifying DDM of all admissions, discharges (out of the NF system) and deceased clients. DDM uses the tracking process to follow PASRR Level II clients as part of the Resident Review process.
Virginia Physical Assessment
A completed Physical Assessment must be submitted with each PASRR referral if a current (within one year) History and Physical is not available (Nursing Home Manual Chapter VI page 8).
