Tracking Form
Use the Tracking Form to notify DDM of all admission, transfers, discharges, and deaths to/from the nursing facility. In the case of a new admission DDM can mail a copy of the completed screenings available for the incoming client. In the event of a transferring client, copies of the NF LOC and Level I should be in the client’s record.
