In 1989, the federal government mandated that states implement a two-phase screening and assessment process for nursing home applicants to and residents of Medicaid certified nursing homes. Originally entitled Preadmission Screening and Annual Resident Review (PASARR), the program was designed to identify and ensure delivery of appropriate services and placements for people with disabilities. PASRR originated on the heels of a report by the Office of the Inspector General revealing that large numbers of previously ‘de-institutionalized’ people had, instead, been ‘trans-institutionalized’ to nursing homes.


PASARR was implemented in 1990 and operated until 1997 without change. While many states recognized the benefits of PASARR, the Annual Resident Review requirements posed undue financial burdens on state resources (ARR mandated a comprehensive annual onsite evaluation of every resident with a disability). The annual evaluation requirement was eliminated in 1997 and the program name was changed to PASRR. All states are required to implement PASRR evaluations and ensure that their programs meet complex and comprehensive federal operating guidelines. This process creates complex support needs such as hiring, training and coordinating clinical teams for onsite evaluations, provider training, database development, reporting, and other support activities.
