Initial Request Form
The Initial Request Form is completed by the social worker to start the Estate Recovery
process. The State Department shall determine whether a medical assistance recipient
reasonably can be expected to be discharged from the institution and return home.
Request For Reconsideration
To appeal the State Departments lien on the client’s estate the client or responsible
party may complete the Request for Reconsideration and submit the completed form
to the Estate Recovery Program.
