Discharge Disposition Form
The Discharge Disposition Form is completed by the social worker to start the Estate Recovery Process. This form is used to determine whether a medical assistance recipient reasonably can be expected to be discharged from the institution and return home.
Request For Reconsideration
To appeal the State Department's lien on the client's estate, the client or responsible party may complete the Request for Reconsideration and submit the completed form to Ascend's Estate Recovery Program.
